I understand that grooming may involve minor risks (e.g., nicks, skin irritation, allergic reactions), and Jess’s Pampered Pups will always prioritize my pet’s safety.

I understand that severely matted coats may require shaving or extended grooming time, which may incur additional fees and could cause temporary discomfort or reveal hidden skin issues.

I understand that the presence of fleas or ticks may result in additional charges or rescheduling to protect other pets.

I acknowledge that for safety, my pet may be gently muzzled or service may be paused. Jess’s Pampered Pups reserves the right to refuse services if behavior poses a risk.

I understand that teeth brushing is an optional add-on service. If selected, it will be performed using pet-safe products and may be skipped or adjusted if my pet shows signs of stress or discomfort. This service supports hygiene but is not a substitute for veterinary dental care, and no guarantees are made regarding plaque or tartar removal.

📋 Vaccination Requirements *

I understand that proof of Rabies, DHPP, and Bordetella vaccinations is required for all grooming services and will be provided prior to my pet’s appointment.

I acknowledge that failure to provide documentation may result in cancellation or rescheduling of my appointment.

🧼 Grooming Services *

I understand that grooming packages vary in services (e.g., bath, brush, nails, ears, styling), and final results may depend on coat condition and pet cooperation.

💳 Payment & Cancellation *

I agree to pay in full at the time of service.

I understand that arriving more than 10 minutes late for drop off may result in a $15 fee or rescheduling.

I agree to give at least 24 hours’ notice to cancel. Missed or late cancellations will be charged a $75 fee. To book future appointments, a non-refundable $75 prepayment is required.

I understand a $15 fee applies if pick-up is more than 30 minutes late, plus $15 for every additional 15 minutes.

🚨 Emergencies *

I authorize Jess’s Pampered Pups to seek emergency veterinary care if I cannot be reached, and I accept responsibility for any related costs.

I understand that while every precaution is taken, Jess’s Pampered Pups is not liable for medical complications related to grooming or pre-existing conditions.

⚖️ Legal Terms *

I release Jess’s Pampered Pups from liability for grooming-related incidents, except in cases of proven negligence.

📸Sharing the glow-up!

I give permission for before-and-after photos of my pet to be used for Jess’s Pampered Pups promotions.
